MONTHLY PAYMENT TERMS
MONTHLY PAYMENT TERMS
Monthly memberships require a minimum commitment of 3 months. Your initial membership payment is due on the membership start date. Each month thereafter, your monthly membership rate (as selected by you) and your 2-week Food Bundle rate (if applicable) will be charged on the membership start date of each month and will continue to automatically renew unless terminated by you in accordance with this Agreement. You are hereby authorizing TANA Lifestyle, LLC (TANA) to process these credit card transactions according to the payment method selected. You acknowledge and agree that the 2-week Food Bundle rate may be subject to change as your plan changes and requires different food products, but a lower rate is not guaranteed. If your Food Bundle rate decreases, you agree that a lower monthly rate may be charged instead of the initial rate at the start of the program. THIS AUTHORIZATION WILL REMAIN IN EFFECT UNTIL THIS AGREEMENT IS TERMINATED OR CANCELLED BY THE PROCESS DETAILED BELOW.
You agree to notify TANA of any changes to your payment information including adding, updating, maintaining, deleting, and verifying the accuracy of any payment information. You agree TANA will not be responsible for any transactions rejected due to erroneous or outdated payment information. You also agree that TANA will not be liable for any use, misuse, lost, stolen or incorrect account or payment information. If TANA is unable to charge your credit card for any reason, TANA retains the right to suspend or terminate your Membership, and reserves the right to terminate your access to automatic payment at any time and for any reason in its sole discretion.
FOOD BUNDLE
Members who choose the 2-week Food Bundle payment option will receive two week's worth of food, according to their plan, for a minimum of 3 months at a 20% discount. Should members miss or reschedule their appointment, the prior order will be fulfilled at the time the 2-week Food Bundle payment is charged. If members do not opt to purchase a Food Bundle, they are required to purchase food a la carte, at the current retail price, according to their plan.
MEMBERSHIP AND FOOD BUNDLE CANCELLATION
To terminate automatic payments, a completed cancellation form must be presented to TANA at least thirty (30) days prior to the next monthly or 2-week auto payment date. Membership and Food Bundle payments may not be cancelled within the first 90 days of membership. Upon receiving the cancellation form, your scheduled autopayments will be cancelled, subject to the notice requirement under this section. Your membership (and all benefits associated with the membership) will continue for the duration of the current month and will terminate the day before your next autopayment would have been scheduled.
REFUNDS
Subject to our Terms of Service and applicable law, all fees and other amounts paid by you to TANA or charged to your credit card within 30 days after you complete the cancellation procedure described in this Cancellation section above are final and non-refundable, including, without limitation, Membership Fees and 2-week Food Bundle payments. You will have access to your Membership through the last date in which you have prepaid.
TERMS OF AGREEMENT
Subject to applicable law and for monthly memberships, TANA reserves the right to increase your monthly membership rate upon thirty (30) days written notice to you. Unless you complete the proper cancellation procedures identified herein, you shall incur the increased monthly membership rate on your next monthly auto payment date following the end of the notice period. You acknowledge and agree that this agreement shall be binding to the fullest extent permitted by law. This Automatic Payment Authorization and your Membership to TANA is governed by these terms. Per consent upon checkout, you agree to be bound to these Terms and Automatic Payment Authorization. If there is any conflict between the terms of this Automatic Payment Authorization and Terms of Service Agreement, the Terms of Service Agreement shall govern and control.
